Many of these people cannot access clean water and electricity, nor pay school fees for their children or see a doctor when they are sick. Their basic needs have been largely unmet, neither by public services nor by the market that doesn't consider them as potential customers. Access to health care services in India by low-income people is limited due to the poor supply from the public service, especially in remote areas such as slums.
